4. Driver's Safety Course Request
   This form allows you to make your request for Driver's Safety Course, but does not guarantee you that you will be granted this option. Your request will be reviewed by a court clerk and you will be contacted by email or by mail once a determination has been reached on your eligibility. If you are granted Driver's Safety Course, a DSC Affidavit will be emailed to you along with your payment requirements. You will have to sign and email the DSC Affidavit back to the court and you will have to pay your fee. You will also need to attach with your online request, a copy of your TEXAS DRIVER'S LICENSE and CURRENT VEHICLE LIABILITY INSURANCE ID card or DECLARATIONS page.

    If the court grants my request for Driver's Safety Course online, I understand and agree that per the provisions of Article 27.14(c) of the Texas Code of Criminal Procedure that I will be entering a plea of NO CONTEST and WAIVING MY RIGHT TO JURY TRIAL. I further understand and agree that in the event that I fail to comply with the terms of my Driver's Safety Agreement, the Judge will find me guilty of the offense and a conviction will be reported to the Department of Public Safety and may appear on my driving record.

    I understand and agree that as part of my online application for the driver safety course (DSC) that I will pay my court costs and fees online or over the phone. I understand that there is a 3.5% fee to pay online and a 5% fee to pay by phone. Once I receive approval from court staff, I understand that I will have 3 days to pay my fees or the online DSC request will be denied and a court hearing scheduled for me to appear on my case.
